To develop and implement a personalised transition plan for CYP with epilepsy for 40% of caseload and map transition care pathways in line with the National Epilepsy Care Bundle.

To establish a pathway for 50% of CYP aged 14-16 to transition CYP’s into adult services by May 2024

To offer transition as a staged approach equitably from 19% to 100% of  our patients aged 16 years and over by March 2022.

East Lancashire NHS Foundation Trust

90% of all 14–16-year-old young with epilepsy will have been sent ‘Ready, Steady, Go’ forms to incrementally educate teenagers regarding their condition and help them develop the necessary skills required for managing their epilepsy independently as young adults by May 2020.
